Patients you can go ahead and contact your provider, schedule an appointment if you haven't had one recently. There are 4 recent blood tests that need to be done so if you haven't had them recently you'll need to. There is a patient consent form you need to sign, here is the link to it if you want to sign and bring with you. It has to be sent in with the doctor form and a prescription.
